ABSTRACT

Background:The neonatal period is the most vulnerable time for a child's survival. As survival perspective of extramural neonates significantly relies on the state of neonate at admission, various survival scores like CRIB, SNAP, MINT, TRIPS, TOPS etc. are designed to prognosticate the outcome of neonate at admission. This study was de signed to delineate correlation of TOPS score with outcome of referred neonates.Aims And Objectives:To recognise correlation of TOPS score with outcomes of extramural neonates. Material And Methods:It was prospective observational study conducted from September 2019 to February 2020 at nicu of tertiary care hospital. Clinico-demographic characteristics and TOPS score were documented in a pre-designed proforma. Hypothermia, Hypoxia, prolonged CRT and Hypoglycemia were defined as<36.5C, <90%, ?3 secs and <40mg/dl respectively. Each parameter was assigned a score of ‘1’ if abnormal and ‘0’ if normal. Neonates were treated as per standard protocol and their outcome was recorded as survive d or expired. Result:Out of 239 neonates 140 (58.6%) were males and 99 (41.5%) were females. The area under the ROC curve (AUROC) for TOPS Total predicting Outcome: Expired vs Outcome: Survived was 0.932. At a cut-off of TOPS Total ?2, it predicts Outcome: Expired with a sensitivity of 83% and a specificity of*The relative risk (95% CI) for Outcome: Expired when TOPS Total is ?2 was 25.68.Hypothermia (Chi-squared test, ?2 = 38.462, p = <0.001), hypoxia (Fisher's exact test, ?2 = 122.636, p = <0.001), poorperfusion (shock) (Fisher's exact test, ?2 = 103.563, p = <0.001) are found to be significantly associated with poor outcome. Conclusion:TOPS score is an easy, reliable and applicable tool for outcome prediction of transported neonates.

ABSTRACT

Background:The corona virus disease pandemic has created remarkable disruption in health system and medical education worldwide. On March 25,2020, the Indian government proclaimed state wise lockdown. During this pandemic, online classes became an educational change from conventional classroom teaching method. Our institute have conducted online classes during this pandemic. Objective of our study was to analyse undergraduate students' perspectives of online classes and compare this to traditional classroom teaching.Material And Methods:The pharmacology department at AMC MET medical college carried out a cross-sectional observational study. A total of 294 students (2nd MBBS and 2nd BDS) took part in the research. Students were given a pre-tested self-designed que stionnaires to analyse the online teaching method.Result:In the present study shows that majority 107(36.3%) of the students believed that the course content provided in online classe s was not helpful. We observed that majority 215(73.1%) of the students were dissatisfied with the practical knowledge provided in online classes. More than half 166(56.4%) of the students believed that online teaching is not helpful in various skill development. A total of 168(57.2%) students agreed to the statement that physical absence of teacher affects both teaching and learning. Only 85(28.9%) students believed that online classes are interactive.Conclusion:From above study we conclude d that majority of students are not ready to adopt online teaching method.

ABSTRACT

Background:Immunization has crucial role in protecting a child from vaccine preventable infections. But during last year 2020, Due to ?COVID-19?pandemic, Routine& supplementary immunization and the surveillance of vaccine-preventable diseases have beenimpacted in many countries. This may lead to the resurgence of vaccine-preventable diseases. So, wehave opted this study to see impact of Covid 19 on routine immunization at vaccination centre of a tertiary care hospital. Aims: To assess effect of covid 19 pandemic on routine immunization. Material And Methods:This study was retrospective observational study, conducted at vaccination centre of tertiary care hospital of Ahmedabad. Data was retrieved from January 2019 to December 2020 from register of vaccination centre and entered in excel sheet and analysed individual month and vaccine wise. Result:Number of immunised children was lower in 2020 as compared to 2019. The result was significant at p<0.05 by Mann Whity test. Inj Td of 10 year was most affected and OPV0 & BCG was least affected. Maximum reduction in percentage was seen in May month and least in February. Conclusion:Health system should design strategies for catch-up vaccination to prevent surge of vaccine preventable disease.

ABSTRACT

Background:Retinal vein occlusion (RVO) is the second most common sight threatening retinal vascular disorder after diabetic retinopathy. Retinal vein occlusion is a common form of retinal vascular disease, especially in middle-aged and older individuals. Occlusion results due to thrombotic effect on the retinal vein . BRVO is classified according to the anatomical location as major or macular. Major BRVO refers to occlusion of a retinal vein that drains one of the quadrants. Macular BRVO refers to occlusion of a venule within the macula. BRVO is further classified into perfused (non-ischemic) or non-perfused (ischemic). Ischemic BRVO is defined as > 5 disc diameters of nonperfusion on fluorescein angiography (FA).Objectives:To study pre and post treatment (Intravitreal Bevacizumab 1.25mg/0.05ml)OCTin patients with macular edema secondary to BRVO. Material And Methods:30 Eyes of 30 patients of BRVO with macular edema were studied retrospectively who attended eye opd during Sept-2012 to Aug-2014 according inclusion and exclusion criteria Afterroutine ophthalmic examination with slitlamp biomicroscopy and dilated fundus examination with indirect ophthalmoscope and preop OCT done for CFT(central foveal thickness)patient underwent intravitreal injection anti VEGF.Posttreatment OCTof these patients were reviewed after 1 week and 1 month retrospectively.Result:Significant improvements in CFT were observed in these patients after treatment. Results were analyzed based on the duration Of the treatment given, after 1 week and after 1 month.Conclusion:Early diagnosis & treatment of the pathology contributes to the better visual outcome.

ABSTRACT

Introduction: The aim of mandibular fracture treatment isto restore the pre-injury anatomic form and function, withparticular care to establish the occlusion. Of the varioustreatment modalities available, Champys miniplate fixation hasbecome the most widely used technique. The 3-dimensional(3D) plating for mandibular fracture treatment is relativelynew. This study was conducted to compare efficacy, stabilityand rigidity of 3-dimensional plates with that of 2mm titaniumminiplates in the surgical management of anterior mandibularfractures.Material and Methods: The study was conducted on 70patients with anterior mandbibular fractures (Symphysis andParasymphysis region). Patients were randomly divided intotwo groups, Group I (n=35) in which the patients underwentfixation by miniplates (2 nos) while in Group II (n=35), 3Dplates were used for fixation. The patients preoperative, intraoperative and post-operative clinical and radiological findingswere recorded in a proforma and were subjected to statisticalanalysis. Fisher’s Exact Test was used to compare the dataobtained from group I and group II patients.Results: Out of 70 patients, 77.14% corresponded to the agegroup of 15-30 years and 82.85% were males. Road trafficaccident (80) % was the leading cause of fracture. The timerequired to adapt and fix the miniplates was slightly more than3-D plates and results were statistically significant. Skeletaland occlusal stability was maintained in both group.Conclusion: There is no major difference in terms of treatmentoutcome in both systems, and both are equally effective inmandibular fracture treatment.
